This culture is to detect the presence of fungi, most notably Cryptococcus sp., in cerebrospinal fluid (CSF) specimens with rapid presumptive detection by an India ink preparation. For other specimen types including body fluids, tissue, respiratory, and others order Unit Code 5164 or for skin, hair, or nails, order Unit Code 5163. Whole blood is not acceptable.
Cultures are held 4 weeks and the test includes identification of the predominant organism or pathogen. A charge will be added for each additional identification. If the isolate appears to be a dimorphic mould, the appropriate DNA probe assays (56500-Blastomyces dermatitidis, 57500-Coccidioides immitis, and 58500-Histoplasma capsulatum) will be performed for an additional charge for each specific probe assay.

Active Q Fever infections are characterized by a fourfold increase in serum IgG between acute and convalescent samples and/or the presence of IgM antibodies directed against phase II organisms. In chronic Q Fever the IgG and/or IgM response is most often directed against phase I organisms, resulting in phase I titers that are greater than phase II titers.
If a screen is positive the appropriate titer will be performed at an additional charge. Titers are not performed or reported if screens are negative. Coxiella burnetti IgA will no longer be performed if the IgG or IgM are positive.

Active Q fever infections are characterized by a four fold increase in Phase II IgG between acute and convalescent samples. In chronic Q fever, Phase I IgG and/or IgM titers are usually greater than Phase II titers. Past infection is usually associated with roughly equivalent Phase I and Phase II IgG titers. The presence of ANA is indicative of numerous disease states including Systemic Lupus Erythematosus (SLE), Mixed Connective Tissue Disease (MCTD), Rheumatoid Arthritis, Sjögren's Syndrome and other rheumatoid diseases. Low titer ANA (<=1:160) may also be seen in the elderly, transiently during viral infection and in otherwise healthy individuals; these ANA titers are of unknown significance. E. coli O157:H7 has emerged as an enteric pathogen of public health importance causing outbreaks of diarrhea, hemorrhagic colitis, and hemolytic uremic syndrome. Although multiple serotypes have been reported to cause disease, the O157:H7 serotype is the most common single strain isolated. Not all O157 strains have the H7 flagellar antigen. This test is performed to confirm identification of the O157:H7 serotype of E. coli. To confirm or detect Shiga-toxin activity of an E. coli O157:H7 isolate, the Shiga-toxin detection EIA is recommended. Please request E. coli, Enterohemorrhagic (EHEC), Shiga-Like Toxin EIA (Focus Unit Code 52068) if toxin detection is required.
